In this fully illustrated psalter, the historiated initials comment on the text of the psalm they illustrate. This initial D begins Psalm 38, "Dixi custodiam vias meas ut non delinquam in lingua mea ..." (I said I will take heed to my ways that I sin not with my tongue), which explains why David is pointing to his mouth in the initial.
